Dear Patient,

To help ensure a smooth and efficient experience for all patients, please review the information below regarding appointments, referrals, cancellations, and clinic policies. If you have any questions, please contact the appropriate department listed below.

Appointments & New Referrals

• To make an appointment, speak to one of our receptionist. You may call between 8AM-5PM on Monday­-Thursday, 8AM-4PM on Friday
• The doctor will only address the concern that you were referred for on your referral
• For new referrals – please allow 2-3 weeks processing time
• Once your referral is received and processed, we will contact you directly to schedule an appointment
• Please request refills from your pharmacy at least one week prior to running out of your medication. It can take up to one week for the doctor to refill a prescription

Cancellations, Missed and Late for appointments

• We require 2 business days notice for appointment cancellation. If you cancel your appointment with insufficient notice, you will be charged a $100 fee
• If you miss your scheduled appointment, you will be charged a $100 fee or the treatment fee for the cosmetic treatment
• If you are up to 1O minutes late for your appointment, it is at the doctor’s discretion whether they can still see you on that day or if it will be considered a missed appointment

Patient Code of Conduct

It is important to us that we create a welcoming and safe environment for all our patients and staff. As such we ask that anyone attending the clinic always behave respectfully towards staff and other patients. If there is behavior that is at odds with this policy, we reserve the right to recommend that you seek care elsewhere. If you are rude or abusive towards a staff member on the phone the call will be terminated.
